Dr. Tim Caraher MD

Dr. Caraher brings a global perspective and a compassionate heart to both of our office locations. Raised in Brazil from ages 4 to 18 as the child of missionaries, he learned early on the value of service, humility, and community. After practicing for eight years in Virginia—including impactful work at a community health center—Dr. Caraher became disillusioned with the limitations of insurance-based medicine. This drove him to relocate to middle Tennessee in 2018, seeking a Direct Primary Care practice model centered around patient wellness.

Dr. Caraher’s practice is rooted in continuity of care. He loves getting to know families, understanding their unique dynamics, and walking alongside them over time as their children grow. His ultimate goal is to educate and empower patients on their individual health journeys, ensuring everyone he treats has a positive, uplifting experience with healthcare.

Raised in a family plagued by obesity and depression, Dr. Caraher ventured into medicine to help others trade unhealth for full and vibrant lives. Following high standing at his medical school and induction into the Alpha Omega Alpha Honor Society, Dr. Caraher chose an unlikely path: community-based Family Medicine. Hoping to understand the root causes of unhealth in America, Dr. Caraher created space in his practice to listen and know to his patients and their families.

He quickly learned how the massive structures of allopathic insurance-based system prevent it from helping patients get well. Initially, Dr. Caraher committed himself to making a difference inside the system. But disillusioned and exhausted, Dr. Caraher eventually discovered how the Direct Primary Care model could empower physicians to prioritize individual patients’ needs and focus onpreventionof disease rather than response to disease. Dr. Caraher intends to use his practice as a springboard to help other physicians and providers break free from insurance-based practices and focus on helping patients and families get well.

Courtney Litton FNP

Originally from Louisville, Kentucky, Courtney brings nine years of diverse nursing experience to our Franklin location. She earned her undergraduate degree from the University of Louisville before moving on to MTSU for graduate school.

Prior to becoming a Nurse Practitioner in 2021, she spent five years at St. Thomas Rutherford, gaining critical experience on the post-surgical floor and spending four years in the cardiac step-down unit. Driven by a passion for education and community, she has also taught student nurses at Columbia State and continues to serve hospice patients on a PRN basis.

Family Nurse Practitioner

Jenni brings a unique blend of deep clinical expertise, wisdom, and an entrepreneurial spirit to our team. After staying home to raise her family, Jenni pursued her passion for nursing in her 30s. She earned her Associate Degree from Columbia State, followed by her Bachelor’s and Master’s degrees from Tennessee Tech.

Before transitioning to private practice, Jenni spent eight years in high-acuity care at St. Thomas Rutherford, ultimately serving as a clinical specialist and team lead on a cardiac, telemetry, and progressive care unit. During the challenges of bedside nursing in the COVID era, she discovered Integrative Direct Primary Care (DPC) during a clinical rotation and knew she had found her true calling.

Jenni Evans FNP

Growing up right here in Spring Hill, Zach brings a wealth of knowledge and a truly adventurous spirit to both of our office locations.

Described by colleagues as a "perpetual student," he has dedicated his life to education and mastering the medical field. He holds an impressive array of advanced credentials, serving as a Nurse, Doctor of Chiropractic (DC), Nurse Practitioner (NP), Family Nurse Practitioner (FNP), and Acute Care FNP. When he isn't collaborating with our team in his spare time, he is on the front lines managing high-acuity cases in the Emergency Room.
